INSTRUCTIONS: READ CAREFULLY
This Request for Tenancy Approval (RTA) may be submitted by the owner or the family. To prevent processing delays, the submitting party must have the 9 digit entity ID for the family. RTAs for an existing owner must also be submitted with the Owner's 9 digit Entiy ID.
Only RTAs for families with active vouchers issued by MDHCV will be processed. Any RTA received for a family that does not have an active voucher will be cancelled upon receipt and will require resubmission upon voucher issuance. NO EXCEPTIONS.
DO NOT resubmit an RTA that is already been submitted through via email, mail, fax, or drop off.
