INSTRUCTIONS: The use of this form is limited to existing participants of the Miami-Dade Housing Choice Voucher (MDHCV) program who have received notice to complete this application. Applications submitted that do not meet this criteria will be ignored.  

To proceed, enter your 9 digit entity ID and the last 4 digits of your social security number. Your entity ID can be found on the top right of any notice from the program.
